Data Sheet
Also called: data collection sheet, datasheet, data form
Definition
The form, paper or electronic, on which behavior is recorded during a session. A good one names the target and its definition, the measurement procedure, the date, the setting and the observer, and it is designed so recording does not interrupt teaching. It is a clinical record and is handled with the same confidentiality as any other.
In plain language
The form where you record the behavior during a session. It should say what you are counting and how, and be quick to fill in while you work.
Example
A sheet with one row per trial, boxes for prompted and independent, and a space for the total at the end.
Not data sheet
A note written from memory that evening. Recording after the fact is not data collection in the sense a plan requires.
