Incident Report
Also called: incident form, critical incident report
Definition
The written record an organization requires after an unusual or serious event — an injury to anyone, a medical event, property damage, a restrictive procedure, an allegation. It is completed the same day, in objective language, by the person who was present, and it is separate from the session note. Employer policy, and often state regulation, set its form and deadline.
In plain language
The form you fill in the same day when something serious or unusual happens, like an injury. Facts only, written by the person who was there.
Example
A learner trips and bruises a knee. The technician completes the incident form before leaving, noting the time, what happened, and who was told.
Not incident report
The routine note describing the day's programs and data. That is the session note.
