What to report, and how fast
Same-day things, and things that can wait for the next contact

What the obligation is
Three of the four documentation tasks on the technician exam are about communication rather than paperwork: passing on what the team raises, asking for clinical direction, and reporting variables that might be affecting progress — each of them promptly. In practice there is a same-day list and an everything-else list. Same day: any injury, however minor; new or escalating behaviour; a caregiver request to change the plan; a crisis or incident; a mistake you made in running a procedure; anything that stopped you delivering the plan; and anything that might be affecting the learner from outside, such as illness, a medication change, or a change in routine. Everything else goes to your regular supervision contact.
In plain language
Report injuries, new or worse behaviour, plan-change requests, your own errors, and outside events like illness on the same day. Save routine matters for supervision.
What this looks like
- Messaging your supervisor before you leave, not at the next scheduled meeting.
- Saying what you observed and what you need, rather than only that something happened.
- Noting in the session record that the caregiver mentioned a new medication.
- Flagging that you could not run part of the plan and why.
Where people get caught
- Waiting for the weekly meeting because nothing seemed urgent at the time.
- Reporting the conclusion rather than the observation, which removes the supervisor's ability to judge.
- Assuming somebody else has already reported it.
- Leaving out your own mistake, which is precisely the thing a supervisor most needs to know.
If you are not sure
Report it. A supervisor can filter an unnecessary message in seconds; they cannot act on something they never heard.
