Continuity of Care
Also called: transition of services, discontinuing services, service transition
Definition
The obligation to plan for the person's ongoing services whenever a practitioner leaves, a program ends, or a client is transferred — giving notice, documenting the plan, handing over to a qualified provider, and not stopping services abruptly. Leaving a position without arranging this is a reportable ethics violation.
In plain language
Making sure the client is not left without services when you leave or a program ends. Give notice, write it up, and hand over properly.
Example
A departing technician gives the agreed notice and reviews the current programs with the incoming technician and supervisor.
Not continuity of care
Quitting by text message the night before a session with no handover.
